What is Primary Care?
Primary Care services provide the first point of contact in the
healthcare system, acting as the ‘front door’ or the NHS. Primary
care includes general practice, community pharmacy, dental and
optometry (eye health) services.
Where more day-to-day health education, advice or treatment is
needed, which could be something urgent, but not so severe that
someone is in critical danger, patients can access a range of vital
local services in person or online, 24 hours a day, 7 days a week.
Where something can’t be dealt with yourself, you can drop into a
local pharmacy, doctors’ surgery, dentist’s or optician’s, for
convenient healthcare from a number of specially trained and
experienced professionals.
What is a Primary Care Network?
Primary Care Networks (PCNs) are groups of practices working together to improve services to their patients. Our PCN is called SASSE 2. It is a collaborative project between Fordbridge Medical Centre, Knowle Green Medical Centre, The Grove Medical Centre, The Orchard Surgery and Packers Surgery.
What are Primary Care Networks trying to achieve?
PCNs are working to make health services more focused around local communities, to improve the way primary healthcare teams work across the area, and to allow GPs and other primary care health workers develop new models of care for their patients and communities.
